03/29/2012 As the Supreme Court justices weigh their decisions on the health care law, some critics are claiming that the justices are crossing the line in a power grab. An editorial in the Washington Post suggests that the court is now acting as an alternative to Congress. It reads, in part: "The conservative justices were obsessed with weird hypotheticals. If the federal government could make you buy health insurance, might it require you to buy broccoli, health club memberships, cellphones, burial services and cars? All of which have nothing to do with an uninsured person getting expensive treatment that others -- often taxpayers -- have to pay for." Judge Andrew Napolitano joined Happening Now to weigh in. He said, "If you are familiar with an appellate courtroom, and this is the ultimate appellate courtroom, you know that judges often throw 'weird' hypotheticals at lawyers to challenge their understanding of the law and to ask where the law might go next." He said that Supreme Court weighs the effects it's decisions will have on future generations and therefore has to look at whether if they allow Congress to force people to buy health insurance, what will they force people to buy next. Judge Napolitano also went on to say that the editorial was probably a direct attack on Justice Scalia, who was appointed by President Reagan. PURPOSE OF REVIEW: To discuss the promising state of the art low-level laser therapy (LLLT) for preventive and therapeutic usage in oral mucositis due to cancer therapy. RECENT FINDINGS: Photomedicine using LLLT is very effective with intraoral and extraoral devices in the management of oral mucositis, based on several studies including randomized control studies. A systematic review identified 33 relevant articles that were subjected to meta-analysis based on which laser parameters in routine practice are being defined. Meta-analysis showed that LLLT reduced risk of oral mucositis with relative risk (RR) 2.45 [confidence interval (CI) 1.85-3.18], reduced duration, severity of oral mucositis and reduced number of days with oral mucositis (4.38 days, P = 0.0009). RR was similar between the red (630-670 nm) and infrared (780-830 nm) LLLT. Pain-relieving effect based on the Cohen scale was at 1.22 (CI 0.19-2.25). SUMMARY: No adverse side effects of LLLT were reported; hence, we recommend red or infrared LLLT with diode output between 10-100 mW, dose of 2-3 J/cm/cm for prophylaxis and 4 J/cm (maximum limit) for therapeutic effect, application on single spot rather than scanning motion. Lesions must be evaluated by a trained clinician and therapy should be repeated daily or every other day or a minimum of three times per week until resolution. There is moderate-to-strong evidence in favor of LLLT at optimal doses as a well tolerated, relatively inexpensive intervention for cancer therapy-induced oral mucositis. It is envisaged that LLLT will soon become part of routine oral supportive care in cancer.

Systems biology and the digital revolution are together transforming healthcare to a proactive P4 medicine that is predictive, preventive, personalized and participatory. Systems biology - holistic, global and integrative in approach - has given rise to systems medicine, a systems approach to health and disease. Systems medicine promises to (1) provide deep insights into disease mechanisms, (2) make blood a diagnostic window for viewing health and disease for the individual, (3) stratify complex diseases into their distinct subtypes for a impedance match against proper drugs, (4) provide new approaches to drug target discovery and (5) generate metrics for assessing wellness. P4 medicine, the clinical face of systems medicine, has two major objectives: to quantify wellness and to demystify disease. Patients and consumers will be a major driver in the realization of P4 medicine through their participation in medically oriented social networks directed at improving their own healthcare. P4 medicine has striking implications for society - including the ability to turn around the ever-escalating costs of healthcare. The challenge in bringing P4 medicine to patients and consumers is twofold: first, inventing the strategies and technologies that will enable P4 medicine and second, dealing with the impact of P4 medicine on society - including key ethical, social, legal, regulatory, and economic issues. Managing the societal problems will pose the most significant challenges. Strategic partnerships of a variety of types will be necessary to bring P4 medicine to patients.

In this study, a natural polymer, chitosan (CS) has been converted through modified procedures to produce a water-soluble nontoxic form that has been evaluated as a novel potential antitumor drug. CS was carboxymethylated and then further modified in mild aqueous medium via graft copolymerization using a new simple and reproducible method. The synthesized new derivative of carboxymethylated CS (DCMC) was fully characterized by numerous techniques including Fourier transform infrared spectroscopy (FT-IR), elemental analyzer (EA), scanning electron microscopy (SEM), two-dimensional wide-angle X-ray scattering (2D-WAXS), and differential scanning calorimetry (DSC). The anticancer activity of the DCMC was investigated using mice bearing Ehrlich ascites tumor cells (EAC) at different doses dissolved in isotonic saline. It has been found that treatment with DCMC significantly inhibited tumor growth in a dose-dependent manner. To better understand the molecular mechanism explaining the DCMC effect on cancer cells, we tested the response of EAC cells in vivo to DCMC using flow cytometry cell cycle analysis. The cell cycle analysis revealed a G₂/M phase accumulation as well as a significant increase in sub-G₁ phase cells after treatment with DCMC. This indicates an induction of apoptosis in EAC cells associated with a highly significant decrease in tumor volume. In general, our results indicated that the DCMC is a regulator of tumor cell growth and differentiation not only by causing G₂/M cell cycle arrest but also inducing their apoptotic death. Moreover, the estimated hematological profile such as hemoglobin, RBCs, as well as WBCs counts revealed normal levels in mice treated with DCMC, indicating the possibility of using the DCMC in cancer chemotherapy without causing anemia like other drugs. Biochemical assays also revealed that treatment with DCMC has led to an augmentation of the antioxidant defense system without affecting lipid peroxidation in EAC-bearing mice.

Paul Ryan appeared on this Tuesday's Morning Joe and defended his latest terrible budget proposal which is more or less a repeat of the last one and that Republicans are apparently going to be willing to support again, despite the fact that once most Americans get a chance to take a good look at what he's proposing, reject the type of policies he's advocating for. Think Progress has done a good deal of fact checking on this and flagged this portion of Ryan's appearance on MSNBC -- Paul Ryan's Budget Includes $3 Trillion Giveaway To Corporations, The Rich: The budget unveiled by House Budget Committee Chairman Paul Ryan (R-WI) this morning includes substantial changes to the American tax code, both for corporations and individuals. Ryan's tax plan shrinks the number of income tax brackets from six to two, with marginal tax rates set at 10 percent and 25 percent. He repeals the Alternative Minimum Tax (AMT), slices the top corporate tax rate to 25 percent, and repeals all of the health care taxes contained in the Affordable Care Act. It also repeals the repatriation tax on profits corporations earn overseas then bring back to the United States. In all, those tax breaks amount to a $3 trillion giveaway to the richest Americans and corporations, according to the Tax Policy Center.
