A total of 80 patients presenting with exogenous constitutional obesity, metabolic syndrome, and diseases of the hepatobiliary system (including chronic non-calculous cholecystitis and fatty hepatosis) were enrolled to participate in the present study. The basal treatment consisted of the adequate reducing diet, remedial gymnastics, massage, reflexo-acupuncture, the application of galvanic muds, and controlled intake of chofitol. It was supplemented with magnetic laser irradiation of selected abdominal regions, and electrical stimulation of femoral, dorsal, and abdominal muscles. It was shown that the combination of the above procedures and physical factors significantly improves the overall outcome of the treatment. The patients suffering intestinal dysbacteriosis were prescribed the intake of probiotic Nor Narine together with Jermuk mineral water; they were found to benefit from such treatment due to normalization of intestinal biocenosis and improvement of their general condition. The clinical and paraclinical data obtained in this study give evidence of the therapeutic efficacy of certain physical factors and especially their combination used for the medical rehabilitation of the patients presenting with constitutional obesity, metabolic syndrome, and digestive disorders. Moreover, the well-apparent positive results were documented from the combined treatment with magnetic laser radiation and therapeutic muds.

The purpose of this study was to test the effectiveness of Korean hand acupuncture (KHA) on bowel symptoms, stress, mental health, and heart rate variability in women with irritable bowel syndrome. From a total sample of 42 women with irritable bowel syndrome diagnosed with Rome III, 21 were randomly selected to be given KHA, whereas 21 were given sham KHA. The KHA group received KHA on 16 spots related to bowel symptoms, whereas the sham KHA group received treatment to areas unrelated to bowel symptoms. KHA needles were applied for 25 minutes, twice a week, for 4 weeks. After the treatment, bowel symptoms were measured using a Bowel Symptom Severity Scale, stress using a global assessment of recent health, mental health using the Symptom Checklist-90-Revised, and heart rate variability. SAS 9.1 (SAS Institute, Cary, NC) was used to analyze the data, and a chi-square test, t test, and paired t test were used for analysis. The KHA group had a decrease in symptoms such as loose stool, bloating, abdominal discomfort, and abdominal pain compared with the sham KHA group (p < .05). However, there were no significant effects on stress, mental health, and heart rate variability. KHA was effective for bowel symptoms such as loose stool, bloating, abdominal discomfort, and abdominal pain, and it could be applied to patients with irritable bowel syndrome. There is a need for further research on the effectiveness of KHA in women with irritable bowel syndrome using more diverse physiological indexes.

OBJECTIVE To observe the changes of the liver blood flow in controlled hypotension by transcutaneous electrical acupoint stimulation (TEAS) combined general anesthesia, thus clarifying the mechanism of liver protection effect in acupuncture anesthesia combined with drugs.Forty-two male beagles were randomly divided into seven groups, i.e. , the general anesthesia group, the 50% control group, the 50% experiment group, the 40% control group, the 40% experiment group, the 30% control group, and the 30% experiment group, 6 in each group. Beagles in the latter six groups were administered with isoflurane inhalation and intravenous dripping of sodium nitroprusside (SNP) for controlled hypotension. The mean arterial pressure (MAP) was lowered to 50%, 40%, and 30% basic MAP and lasted for 60 min. Beagles in the general anesthesia group was not treated with controlled hypotension. In the experiment groups, TEAS was applied to bilateral Hegu (LI4), Zusanli (ST36), Sanyinjiao (SP6), and Quchi (LI11) at 2/100 Hz with the stimulation strength of 4 +/- 1 mA. The TEAS started from the stability of physiological conditions to 60 min after maintaining the target MAP. The changes of blood flow of the liver tissue surface at corresponding time points were monitored by laser Doppler blood flow meter.Between the beginning of hypotension and the maintaining stage of target low blood pressure, the liver blood flow of the 50% control group was significantly lower than the level of the general anesthesia group and the basic level at corresponding time points (P < 0.05). It was significantly reduced in the 50% experiment group only at 30-60 min of maintenance. Besides, in the early period of maintenance (10-30 min), it was significantly higher in the 50% experiment group than in the 50% control group at the same time points (P < 0.05). In this stage, there was no obvious increase in the liver blood flow in the 40% and 30% experiment groups. In the recovery phase of blood pressure (20-30 min), the liver blood flow of the 40% experiment group had been restored to the level of the 40% control group and the basic level, while it had not been restored in the general anesthesia group. In this stage, the similar changing tendency of the liver blood flow occurred in the 50% and 30% experiment groups and the 50% and 30% control groups.Line to a high level of controlled hypotension (50%), TEAS liver protective effect was obviously embodied in the early step-down phase and the maintenance phase. Line-induced hypotension to a lower level (40%), TEAS liver protective effect was obviously embodied in the recovery phase.

The purpose of this study was to evaluate the effect of laser acupuncture (LA) and soft cupping on low back pain. In this study, the subjects were randomly assigned to two groups: active group (real LA and soft cupping) and placebo group (sham laser and soft cupping). Visual analog scale (VAS) and Ryodoraku were used to evaluate the effect of treatment on low back pain in this trial. Laser, 40 mW, wavelength 808 nm, pulse rate 20 Hz, was used to irradiate Weizhong (BL40) and Ashi acupoints for 10 minutes. And the Ryodoraku values were measured 2 times, that is, before and 15 minutes after treatment. The results show that there were significant difference between the first day baseline and the fifth day treatment in VAS in the two groups. Therefore, LA combined with soft cupping or only soft cupping was effective on low back pain. However, the Ryodoraku values of Bladder Meridian of the placebo group have been decreased apparently, and didn't come back to their original values. It means that "cupping" plays the role of "leak or purge" in traditional Chinese medicine (TCM). On the other hand, the Ryodoraku values of Bladder Meridian of the active group have been turned back to almost their original values; "mend or reinforcing" effect is attributed to the laser radiation.

Neuropathic pain is a complicated symptomatic disease as migraine in recent years. Not because the pain character differed from the nociceptive inflammatory symptoms but because of its complexity of mechanisms. Though peripheral sensitization, ectopic discharge, central sensitization, central re-organization and loss of inhibition play part of roles in mechanisms, however, based on this mechanistic treatment, the outcome still disappointed physicians and patients, exampled as central post-stroke central pain (CPSP). The pain reduction is far less than the expectation from patients and physician's under-treatment frequently occur due to the fear of adverse effects or off-label use of these anti-neuropathic pain drugs. Therefore, a multidisciplinary procedure including non-pharmacological management, rehabilitation program, careful explanation, stepwise pain reduction, daily diary record, and tailored individual planning for medications are helpful in treating this kind of sufferers. Pharmacological treatment is the mainstream in post-herpetic neuralgia (PHN), diabetic peripheral neuropathic pain (DPNP), central post-stroke pain (CPSP), trigeminal neuralgia (TN), complex regional pain syndrome (CRPS), cancer pain, failed back syndrome etc, while polypharmacy is still the major prescriptions facing such kind of miserable patients. The tricyclic antidepressants (TCA), gamma- aminobutyric acid (GABA), voltage-dependent calcium channel blockers, selective non-epinephrine reuptake inhibitor (SNRI), opioid or morphine etc, are still evidence-based medicines (EBM) but with different outcome for individuals. Acupuncture is to some extend effective in Taiwanese people with perceived evidence or placebo. The Taiwan guidance for total pain management and review of EBM in treating neuropathic pain from neurological point of view will be introduced in this manuscript.
