Objective:The Mental Health Intervention Project (MHIP) was established in Queensland in 2005 to improve interagency collaboration in the management of mental health crisis situations. The aim of this present study was to assess changes in the use of Emergency Examination Orders (EEOs) by police and ambulance officers since the commencement of the MHIP.Method:Information concerning the use of EEOs was analysed to highlight changes that have occurred since the inception of the MHIP.Results:The use of EEOs by police and ambulance officers increased by 262% since 2002. The proportion of EEOs completed by ambulance officers has increased from 14.5% in 2004 to 38% in 2010. Involuntary assessment was deemed necessary for less than half of the individuals presented by police and ambulance officers.Conclusions:Police continue to generate two-thirds of all EEOs completed. Police and ambulance officers may benefit from additional training in the use of EEOs. Providing alternative accommodation/treatment options for those under the influence of alcohol and other drugs may help to reduce the inappropriate use of EEOs and subsequent demand for Emergency Department services.

To observe the effect of electroacupuncture (EA) intervention on expression of signal transducer and activator of transcription 3 (STAT 3) in the focal ischemic cerebral tissue, so as to study its mechanism underlying improving ischemic stroke.A total of 150 SD rats were randomized into sham operation (control) group, cerebral ischemia (CI) model (model) group and EA group which were further randomly divided into 2 hour (2 h), 1 day (1 d), 3 d, 1 week (1 W) and 3 W subgroups (n = 6/subgroup for immunohistochemistry, n = 4/subgroup for Western blot). CI model was established by occlusion of the middle cerebral artery with electro-coagulation method. EA (3 Hz/20 Hz, 2-3 V) was applied to "Baihui" (GV 20) and "Dazhui "(GV 14) for 30 min. The expression of cerebral STAT 3 was detected by immunofluorescence histochemistry and laser-confocal microscopy, and Western blot, separately.Compared with the control group, cerebral STAT 3 immunofluorescence intensity values at the time-points of 2 h, 1 d, 3 d and 1 W, STAT 3 protein expression levels at the time-points of 2 h, 1 d and 3 d in the model group were increased significantly (P < 0.001, P < 0.05). After acupuncture intervention, cerebral STAT 3 immunofluorescence intensity values at the time-points of 1 d, 3 d, 1 W and 3 W, STAT 3 protein expression levels at the time-points of 1 d, 3 d and 3 W in the EA group were down-regulated considerably (P < 0.001, P < 0.01, P < 0.05). No significant differences were found between the control and model groups in STAT 3 immunofluorescence intensity at 3 W, and in STAT 3 protein expression levels at 1 W and 3 W, and between the EA and model groups in STAT 3 immunofluorescence intensity at 2 h, and in STAT 3 protein expression at 2 h, 3 d and 1 W (P > 0.05).EA therapy can down-regulate the expression level of STAT 3 protein in the regional ischemic cerebral tissue in cerebral ischemia rats, which may contribute to its efficacy in the treatment of acute and chronic ischemic stroke.

Physiotherapy as bricolage: Theorizing expert practice.

Theories about how knowledge is sought and applied in clinical practice are often referred to as practice epistemologies, and have not been extensively explored in the physiotherapy profession. Tacit assumptions about what counts as physiotherapy knowledge thus form the basis for many approaches to gaining and using information in practice. The purpose of this paper is to propose a physiotherapy practice epistemology, through the notion of the bricoleur, which takes an alternative approach to understanding how knowledge might best be viewed in relation to physiotherapy. The term bricoleur refers to a handyman or handywoman who uses all tools and types of knowledge available. The notion of physiotherapists as bricoleurs recognizes that all practice knowledge is situated within social, cultural, and historical contexts that shape our beliefs about what counts as physiotherapy knowledge. This recognition leads physiotherapists who act as bricoleurs to embrace multiple epistemologies, discovering new ways of knowing and clinical reasoning strategies to provide a more holistic approach to physiotherapy practice. The relationships between expertise in clinical reasoning and the epistemology of the bricoleur are then addressed, explicating the utility of multiple epistemologies in achieving excellent physiotherapy care. A bricoleur's epistemology is then applied to the concept of expertise in physiotherapy, de-stabilizing the notion that a single authoritative approach to the practice of physiotherapy ought to be idealized.
